Home Health Value-Based Purchasing Series: Measuring Performance Improvement

Course description

VNAA and the Alliance for Home Health Quality and Innovation have teamed up to provide a webinar series focused on Home Health Value-Based Purchasing.

As the CMS Value Based Purchasing program kicks in, agencies will need to adopt new programs and initiatives to drive quality. Please join speakers Andrea Foley, MSN, RN, CWCN, Director of  PI/Education, NVNA & Hospice, Suzanne Clark, PT, DPT, GCS, COS-C, Staff Development Coordinator, NVNA & Hospice and Liza Greenberg, RN, MPH, Interim Vice President, Quality and Performance Improvement, VNAA as they discuss clinical and operational best practices that will help agencies improve their performance in the HHVBP Model. Each agency will target performance improvement to the topics that most impact patient outcomes and performance measures.  Quality improvement strategies may range from improving medication reconciliation practices to developing partnerships that reduce hospital readmissions.  Best practice recommendations will be drawn from the VNAA Blueprint for Excellence.
